This can happen when you try to load php5.dll compiled for Apache x64 while your php is x86, and vice versa. always check if the vendor has provided some sort of removal/cleanup tool/utility (try different combinations of words to make sure you find it). I think the only way I'm going to bother is if someone develops a windows installation package. PHP was not in the PATH either, so looked into Apache bin/ directory and voila, php5ts.dll was right there.
